Article 12 the application food business license, shall be the place where the applicant is located the local food and drug supervision and administration departments at or above the county level shall submit the following documents: (
A)
Food business license application form;
(

The business license or other subject qualification certificate copy.
(
3)
Appropriate to the main equipment and food business facilities layout, operation process and other documents;
(
4)
Food safety check list, workers, health management, incoming inspection records, food safety accidents disposal, etc to ensure the food safety regulations.
Using vending equipment engaged in food sales, the applicant shall also submit certificate of products vending equipment, specific place, the operator name, residence, contact information, food business license very method etc.
The applicant entrust others to handle food business license application, the agent shall submit a power of attorney and the agent's identity documents.
